When death occurs at the hospital
Most hospitals will provide a CCOD (Certificate of Cause of Death) upon death.
Upon receiving a CCOD, your family may engage a funeral director for the following procedures​. After which, your family should take the CCOD and exchange it for a Death Certificate.

A Death Certificate is required for booking of a cremation at certain Columbariums and etc. However, not all hospitals provide a death certificate. If it is not provided, the deceased's Family member in charge will have to head down to the nearest police station to exchange it for a death certificate.
When death occurs at home
Contact your family doctor or a neighbourhood doctor who is able to certify the death of your loved one. ​If unable to contact one, you may also contact your funeral director for assistance.
A CCOD will be issued on the spot if the doctor is able to certify the cause of death of the deceased. If he is unable to certify the cause of death, you will have to report to the police to arrange for the body to be sent to Singapore General Hospital's mortuary, located at Block 9.
